Duval Volleyball League - Liability Waiver & Media Release

Welcome to Duval Volleyball League!

By completing registration and submitting payment, you acknowledge and agree to the following terms:

1. Voluntary Participation and Assumption of Risk

I am voluntarily participating in the Duval Volleyball League. I understand that volleyball is a physical sport involving inherent risks such as falls, collisions, contact with other players, or equipment-related injuries. I knowingly accept and assume all such risks, whether foreseen or unforeseen. Players must be 18 years of age or older to join the league. By registering, you confirm that you meet this age requirement.

2. Release and Waiver of Liability

To the fullest extent permitted by law, I hereby release, waive, and discharge the League, its organizers, volunteers, staff, sponsors, facility owners, and affiliated entities from any and all claims, demands, actions, or causes of action arising from injury, loss, or damages I may sustain in connection with my participation in League activities, whether caused by negligence or otherwise.

3. Medical Authorization and Responsibility

I understand that the League does not provide medical insurance or medical personnel on-site. In the event of an injury or medical emergency, I authorize trained emergency responders or licensed medical professionals to administer necessary medical treatment. I understand that I am solely responsible for any medical expenses incurred as a result of my participation in League activities.

4. Personal Conduct and Compliance

I agree to follow all League rules, including the Duval Volleyball Association Corp.'s Code of Conduct, which outlines expectations for behavior, sportsmanship, and respect toward players, officials, and staff. I understand that violations of the Code of Conduct may result in disciplinary action, including suspension or removal from the League without refund.

5. Media and Photography Release

I understand that League events may be recorded through photographs, video, or other media. By participating, I grant the League, its affiliates, and authorized media partners the right to record, use, reproduce, and distribute images, video, and audio of me taken during League activities for any lawful promotional, advertising, or editorial purposes, in any format (print, digital, social media, website, etc.) without compensation.

This release is granted in perpetuity and includes the right to edit or modify such content. I waive any rights to inspect or approve the final materials. If I do not wish to be recorded or photographed, I will notify the League in writing prior to participation.

6. Binding Agreement and Jurisdiction

This waiver is a legally binding agreement and applies to me, my heirs, executors, and assigns. It shall be governed by the laws of the State of Florida. If any provision of this agreement is found unenforceable, the remaining provisions shall remain in full force and effect.

7. Payment and Refund Policy

All registration fees must be paid in full prior to participation. Refunds are not guaranteed and are issued only at the discretion of League organizers in cases of event cancellation or other special circumstances. Players removed from the League for Code of Conduct violations will not receive a refund.

8. Agreement and Acknowledgment

By checking the box during registration and submitting payment, you confirm that you have read, understood, and agree to these Terms and Conditions, including the Liability Waiver, Media Release, and Code of Conduct. This agreement is legally binding and governed by the laws of the State of Florida.
